  • The cost for embroidering is $36 per name.  
  • The names will be in Hebrew script. The format is first/middle Hebrew name plus a transliteration of the last name. The name will be embroidered exactly as presented in the form below so please print clearly.
  • As possible, based on the design and room on the tallit, names will be grouped together if submitted together. (Note that names submitted in future years as part of this annual initiative cannot be guaranteed a location near previously submitted names.)
  • Names must be submitted, with payment, no later than Friday, August 29, 2025 for inclusion in this year’s initiative.
